Graduate Certificate in Applied Data Science: Certificate Completion Application

The certificate completion application should be submitted during the second half of the semester in which you complete the certificate requirements. All three courses must be either completed or currently in progress.

• Students graduating at the end of spring must apply by May 1.
• Students graduating at the end of summer must apply by August 1.
• Students graduating at the end of fall must apply by December 1.

If you are a current student and:

• ... you have already completed the three courses for the certificate:
  • Please submit this form now (before the deadlines listed above).
     
• ... you are currently taking your final course(s) for the certificate:
  • Please submit this form in the second half of the current semester (during or after week 8 of instruction).
     
• ... you need to take at least one certificate requirement in a future semester:
  • Please wait to submit this form until you are currently taking or have completed all three certificate requirements.

Step 1: Add the Certificate in CalCentral

In order to be awarded the certificate, all students must add the certificate plan (also referred to as “declaring the certificate”) on CalCentral. The request must be approved by an advisor in your home graduate program.

After following the steps on the instruction page, contact your graduate advisor to let them know you've submitted the request and you need them to approve it.

Step 2: Submit the Completion Form Below

After the semester is complete and final course grades have been entered in your academic record, staff will verify whether you have successfully met the certificate requirements. The certificate completion will be noted on your official transcript, and a physical certificate will be mailed to you. Certificates are typically mailed within 2–3 months of the end of the semester. The certificate will not appear on your UC Berkeley diploma.
